Prior to your first appointment you will need to download, fill out, sign, save and email these forms: NLT-Fillable-Forms. You will need to use Adobe Reader to fill out and sign these forms. You may download the software here: Adobe Reader. For more information on creating and using digital ID’s Visit this site: Adobe Digital ID’s
You will also need to print out this section of forms as they are needed in hard copy form: NLT-Print-Forms
Please ensure that you have sent in the digital forms as well as have all printed forms ready on your first appointment. Please remember to bring a copy of your insurance card.
Also, Please visit Kids and Horses Website to fill out the “Client Application Forms” and “Doctors Release” Forms prior to your first appointment.
